Our Cardiac Rehabilitation Program helps patients with heart disease
to return to full and productive lives. This comprehensive, medically
supervised program includes exercise, education, counseling and cardiac
risk factor modification. It is designed to limit the physiological and
psychological effects of cardiac illness, control cardiac symptoms,
stabilize or reverse the atherosclerotic process, and enhance the
psychosocial and vocational status of the participating
patients.
CareMaps
An integral part of our comprehensive, collaborative care, CareMaps ensure
that your care is efficiently coordinated throughout the hospital, and that
your treatment plan is communicated effectively to you and your
family.
CareMaps are customized individually for every cardiac patient upon admission
by a multi-disciplinary team of physicians, nurses, physical therapists,
respiratory therapists, cardiac rehabilitation staff, social services staff,
nutritionists and spiritual advisors. Shared with patients and family, CareMaps
provide an easy-to-understand outline of what to expect during your
stay.
For instance, a patient scheduled for open-heart surgery would begin with
pre-admission, where a registered nurse offers pre-operative education and shares
an individualized CareMap with the patient and family. The CareMap outlines
admission to a telemetry unit, where preparation or surgery occurs and progresses
through surgery to intensive care and then to the Acute Observation Unit (ACU),
where in-patient cardiac rehabilitation begins.
Preliminary discharge plans are included in the CareMap, based upon home
environment, support persons, access to community services, and details of medical
health and history. Often, a patient will be referred to hospital-based home care
through TrinityCare, another of PLCMs affiliates. Discharge planning is
regularly updated based upon the patients condition.
Providence Little Company of Marys Cardiac Rehabilitation program is a series of three
phases designed to help the patient who has been diagnosed with heart disease or
is recovering from heart surgery to lead a more healthy and
fulfilling life.
