Transitional Care

As you progress through the acute care phase of your illness, your physician may determine that you are ready to be discharged from the hospital, but not yet ready to return home.

Transitional care is designed for patients who require a short-term regimen of less intensive therapy or treatment following an acute hospital stay. It is ideal for patients who are too ill and/or technology-dependent to receive home care, but who do not need to remain in a medical/surgical bed. Our hospitals’ Pavilions and the Transitional Care Unit at the Beach Cities Ambulatory Care Center offer the experienced, multi-disciplinary care needed to make the transition from hospital to home.

Home Health

Hospital-based home care for cardiac patients is available through Providence Little Company of Mary Home Health. Using a multi-disciplinary approach, Providence Little Company of Mary Home Health brings together a full range of health care services for delivery to patients in the comfort of their own homes. Providence Little Company of Mary Home Health can help cardiac patients recovering in a home environment with everything from physical rehabilitation to intravenous therapies.

The plan of care and treatment is determined and updated continually by the attending physician, appropriate team members and the patient and family members.

Case Management

To help facilitate the transition from hospital to home, patients with chronic cardiac problems or who have undergone open heart surgery enter a Case Management Program. After returning home, patients meet regularly with a Case Manager who makes sure they have the proper monitoring equipment and supervises every step of their self-management progress. Case Managers formulate, communicate and support an individual plan of care through hospitalization to recovery and home.
