The Rehabcentre’s goal is to restore the patient to the maximum level of independence possible. As part of the Providence Little Company of Mary family we are committed to caring for the entire person — mind, body and spirit. The majority of patients admitted to the Rehabcentre are discharged to home with continuing therapy. The Functional Independence Measure (FIM) is used to monitor patient progress against established goals.

 
In addition to assisting the patient’s transition from hospital to home and community through support groups, home assessments and community reintegration, the Rehabcentre staff also maintains communication with the patient’s personal or primary care physician throughout the length of stay.

After discharge, follow-up appointments are scheduled with the primary care physician or specialist, and a two-week follow-up visit is planned with the rehabilitation physician. This ensures that the patient is enrolled in the recommended outpatient or home health program and provides the continuity necessary for successful rehabilitation. Other services available at the Rehabcentre include on-site outpatient physical, occupational and speech therapy.

The Rehabcentre is Joint Commission on the Accreditation of Healthcare Organizations – accredited, Medicare certified and CARF accredited for comprehensive inpatient medical rehabilitation. We have been licensed since 1981, and are the sole acute rehabilitation provider in the South Bay and Harbor areas of Los Angeles.
